When we opened the old, existing profile it didn’t ask for ANY authentication, it just started working. Yeap, you describe it exactly and unfortunately we still don’t have an answer. Closed the window without entering anything and Outlook returned to ‘Connected to Exchange’. We are looking at using conditional access policies where a user with a Domain joined PC is not prompted for MFA.
No prompt for MFA is happening in 365 apps Outlook desktop. OWA says MFA is satisfied. ONE user only.

You can skip it for 24 hours at a time, but after seven days from the first notice you'll have to verify or add new security info before you can sign in again. In order to use Authenticator on Android for your work or school account, push notifications for the app must be enabled and Google Play Services and the Google Play Store must be downloaded and enabled. I created a new Outlook profile, and then the old one started working. I wonder if the compuer cached a new logon token or something that the old Outlook profile could use. Also, during the new profile creation, I was only prompted for a password and not for MFA.
Find solutions to common problems with MFA if you are having trouble approving your sign in requests
If you suspect someone else is trying to access your account, contact your IT administrator. The error could be caused by malicious activity, misconfigured MFA settings, or other factors. To investigate further, ask an IT administrator to look at the Microsoft Entra sign-in logs.
I can't sign in after multiple attempts
In each case the fix has either been one of the above items or a combination of them. There doesn’t appear to be any pattern to the issue occurring or the problem being limited to any specific computer or user. When Exchange connectivity diagnostic I switch from company's Teams to client's, I am asked to log in. The log in prompt shows a code and to authenticate it in Microsoft Authenticator.
You are using Email Token Multi-Factor Authentication
Unfortunately when the problem occurs they’ve asked us to ring straight away but of course you can never get them instantly (we’ve just been given someone’s direct number now). To make a long story short we’ve since been asked to collect Outlook logs and install Fiddler to capture additional logging. We strongly recommend letting your organization's Help desk know if your phone was lost or stolen. After your settings are cleared, you'll be prompted to register for two-factor verification the next time you sign in.
Downloading the Authenticator App for Android devices in China
We too have been seeing this more and more, all W10 on O365, although we use Okta as our MFA. I feel like this issue is getting progressively worse as the fix used to be clearing the credential manager or if it was bad uninstalling the entire Office Suite, clearing all traces and reinstalling and now we are seeing nothing helping Fix Outlook Not Opening After Windows Update it. Have looked all over and people have all sorts of possible solutions, but nothing we found to resolve the issue and get a new issue daily and just keep going through until it eventually works.
Starting February 2026, we will introduce Jailbreak/Root detection for work/school Entra credentials in Microsoft Authenticator. This change enhances security by preventing Authenticator Entra credentials from functioning on jailbroken or rooted devices. This will forget all Wi-Fi networks and passwords, mobile settings, Virtual Private Network (VPN) and Access Point Name (APN). Microsoft may limit repeated authentication attempts that are performed by the same user in a short period of time. Waiting ten minutes before trying again will usually resolve this, if not you should contact the IT Service Desk for support.
Security verification messages are sent in the language of your account when the security info email/phone number was added. We won't ask you to verify your security info every time you sign in. Occasionally, Outlook OST vs PST repair we might ask you to check it again—for example, if you haven't signed in for a long while. Problem doesn’t just happen on PC’s connected to the internal network, but can happen when a laptop is remote and connected via external wifi or mobile hotspot.
